For some reason I can't get the text input from the form. I'm successfully able to get the file.

Your second input overrides the first. Try to make a:
var_dump($_POST);
at the beginning of the php script, so you will see what is arriving as a parameter

If I change enctype="multipart/form-data" into form tag, it's ok, but I need this tag.
You still need the enctype attribute, as the files will not be available without it.

instead of

$_FILES['file'] contains array of properties of uploaded file. use print_r instead. It will work fine.

Use a validator: You misspelled enctype (it has a c in it).
Consequently, the form is being submitted with the default (url based) encoding which doesn't support file uploads.
You need to think that as a two step page. First, you send your form, then you use the input.

For renaming a uploaded file, it better to upload it to server with move_uploaded_file and than rename it.

What you're trying to do here, if I'm not mistaken, is upload images on your server, right? You can simply use a third-party upload tool for that. I'd recommend AjaxUpload as it is based on AJAX and pretty easy to implement.
For simplicity, I'm applying the upload functionality on one image. You can simply put it inside a function and reuse it.

I have a single profile page that I want to upload a photo on as a separate action. I have the first form submitting to the page successfully, it is when I submit the photo form that the page returns blank with an empty message.

When the "btnSubmit" is called the page loads and the textbox gets the name that was entered. When the "btnUploadPhoto" is called I get this on the screen and nothing else:

As you are uploading a file, you will want to check if a file has been submitted in your PHP code. Here is the code to do this:

I have the following form:

The php works as a proxy and is OK (i do have to change the POST method to PUT in the proxy).
When upload is finished, the page turns blank and the path i see in the browser is the path to the php.
