I've been programming in PHP for several years now and never encountered this error before.
Here's my widget.php file:
require_once('fruit.php');
echo "I am compiling just fine!!!";
And my fruit.php file:
$bVar = true;
When these two files look like this ^ then everything compiles with no errors and I get the "I am compiling just fine!!!" success message.
Now, the minute I move the fruit.php file one directory level up, and change my widget.php file to reflect the directory restructuring:
require_once('../fruit.php');
echo "I am compiling just fine!!!";
Now all the sudden, I get PHP warnings & fatal errors:
Warning: require_once(../fruit.php) [function.require-once]: failed to open stream: No such file or directory in /webroot/app/widget.php on line 1
Fatal error: require_once() [function.require]: Failed opening required '../fruit.php' (include_path='.:/usr/local/php5/lib/php') in /webroot/app/widget.php on line 1
In all my years working with PHP, I've never seen require_once() fail like this before. Any ideas?!?!
Maybe you are in the wrong work directory. Its a bad idea to rely on it (except you explictly want to access it) anyway. Use
require __DIR__ . '/../fruit.php';
or with pre-5.3
require dirname(__FILE__) . '/../fruit.php';
Remind, that paths starting with .., or . are not resolved against the include-path, but only against the current work directory.
Remember that in the second case, you're specifying a path, but in the first case it uses your includes path. Perhaps rather than explicitly specifying .. in the second case, you case modify your include path.

In my experience, the "open_basedir" directive is most likely to cause this issue.
require_once('fruit.php');
This searches for fruit.php in the same directory as widget.php is in, no matter what the current working directory is.
require_once('../fruit.php');
This searches for fruit.php in a directory above the current directory, not in the directory above the one widget.php is in.

Based on the error posted, it seems to be that the include_path is referring to the path (include_path='C:\xampp\php\PEAR') while the PHP files are included in some other folder.
You need to update the include_path variable to point to the correct location. You can find this in the php.ini file.
Computers don't lie, if it says it doesn't see it, then it is not there
Make sure you are making it look at the right place, and if I will advise you do it manually also to confirm
I also suspect it is an issue arising from using relative paths. Please make sure the path is right considering where you are currently calling the file from
e.g calling include(../includes/db.php) from index.php is different from calling it from admin/index.php

If your are calling from C:\xampp\htdocs\portfolio\admin\includes\admin_header.php to C:\xampp\htdocs\portfolio\includes
then
include(../../includes/db.php);
is the right syntax to use

I have had this the same issue before and it ended up being something dumb on my part i forgot to start the MySQL on the XAMPP control panel. I'm sure i'm late the the party on the answer but hope it can help someone later down the road.
a bit late but i just reply for those who might get the same error .
assuming you are storing header.php in your includes directory and its not beside your index.php,so when you include bd.php into your header.php and then including your header.php into your index.php pay attention that apache knows index.php direcotroy as your pwd so if you have set that relative path for db.php based on header.php directory its not gonna work.you can use absolute path to avoid this or just set you relative path based on relative location of db.php from index.php
It look likes, the files you are trying to include in your php codes, are not on that place. Try to save all your include files in a particular folder and then call it wherever you want to call it.
suppose your index.php file is in xampp/htdocs/your_folder_name/index.php , then simply create a folder name whatever you want to name it inside your folder name your_folder_name/your_new_folder and save all your files which you want to include, as if you are saving db.php then call it on the top of your page as:
<?php include("your_new_folder/db.php");?>
This has to do with how you are coding your relative path.
I am assuming you got this error when your app made reference to admin_header.php, which has an include() to try and call {document_root}/porfolio/admin/includes/db.php. My educated guess is it contains your implementation of the connection string to the database.
The correct relative path should be: include('../../includes/db.php');

In case this helps anyone else, I had this problem and then realised WAMP had crashed (or maybe I didn't start it). Either way, WAMP wasn't running on my local machine, so... doh.
This should be the very first thing you check (in line with, "is it plugged in?").
Try removing relative paths ("../")
e.g. change include(../includes/db.php) to include(includes/db.php)

Notice how it says it can’t find inc/connect.inc.php in this directory:
C:\xampp\htdocs\taximat\inc\coreturni
It’s because your require statements are referring to relative paths. So when the file list.inc.php is loaded, the require is assuming that inc/connect.inc.php is located in C:\xampp\htdocs\taximat\inc\coreturni. Which is why it is failing.
You should set a base path in your app like so. Just a note that I am unclear on forward-slash or back-slash syntax in a case like this since I work mainly on Mac & Unix systems and the error shows windows paths, but the PHP is showing Unix paths.
$BASE_PATH='/xampp/htdocs/taximat/inc/coreturni/';
And the require all files like this:
require($BASE_PATH . 'inc/coreturni/list.php');
And like this:
require($BASE_PATH . 'inc/connect.inc.php');
That way no matter where the files are called from, the require will use an absolute path to the file. And since $BASE_PATH is a one-time variable you set you can just set it in your main config file and not worry about it. To make the code portable, just change $BASE_PATH based on system setups.
When the page is displayed for the first time, your paths are like this:
page.php
inc/
coreturni/
list.php
connect.php
In other words, all require statements will be relative as seen from page.php.
However, when you request list.php directly using AJAX, it looks more like this:
list.php
../
connect.php
Now, the require should have been for ../connect.php.
Solution
One way to solve this is by calculating the absolute path based on the file currently being processed (i.e. list.php) like this:
require dirname(__DIR__) . '/connect.php';
The dirname(__DIR__) construct will always yield the directory on your file system that's one higher up than the current script is in.
I'm trying to include a file (/wp-load.php) at the beginning of the /html/ directory. I'm trying to include it from /wp-content/themes/pw-steel-orange/index-load.php, but I always get the error message

You can issue the following command to see where you are pulling the file from (where you are at):
// get your current working directory
echo getcwd();
Then include the file accordingly.
// file is 3 dirs back
include '../../../wp-load.php';
If you are using a framework like CodeIgniter, for instance, there will be an index.php file at the very start of your application that will be calling all other code. So you would have to include that file according to that file.
Most frameworks use something they call a BASEPATH that is the current actual full server path to the site. This may prove very useful when migrating your site to another destination.
Here is a semi-automated way to do this:
$incPath = str_replace("/wp-content/plugins/PLUGIN_NAME","",getcwd());
ini_set('include_path', $incPath);
include('wp-load.php');
Regardless though, it's still a bad idea to include wp-load.php. ( if this link is ever deleted, See that page here )
When I use ../mysqlConnect.php I get the following messages.

When I use the directory name - mydir/mysqlConnect.php - everything works fine.
require_once('../mysqlConnect.php') asks PHP to look in the directory above the one your script is currently in for mysqlConnect.php.
Since your connection file appears to be in a mydir directory, require_once('mydir/mysqlConnect.php') works because it looks in that directory, which is contained by the one it's currently in.
Visual representation (assuming script.php is your script including that file):
dir/
subdir/ # PHP looks here for ../mysqlConnect.php
script.php
mydir/ # PHP looks here for mydir/mysqlConnect.php
mysqlConnect.php
Require is relative to the invoced script, not the script you call require() in. Use something like this to have an absolute path:
require(dirname(__FILE__) . '/../mysqlConnect.php');
In PHP 5 you can also use DIR.

That's because you are not specifying the correct include path. ../ refers to parent directory. ../../ goes two directories back, ../../../ goes three of them back. If the mysqlConnect.php file is present in the same folder as your script, you don't need to specify ../ in the include.
Make sure that you specify the correct path. You can easily check whether or not you are specifying correct path like:
if (file_exists('../mysqlConnect.php'))

echo 'Iam specifying the correct path !!';
}
else
{
echo 'Well, I am not :(';

require(_once) and include(_once) work relative to the path of the first script, i.e. the script that was actually called.
If you need to require something in an included or required file, and would like to work relative to that file, use
dirname(__FILE__)."/path/to/file";
or as #Arkh points out, from PHP 5.3 upwards
__DIR__."/path/to/file";
