I had the following form working properly and populating MySQL DB. I had since added on a new field 'type' to my database and also added it to the form. However now when I try to add a new entry it says "Invalid column name 'type'". Any help would be greatly appreciated.

You will need to escape the column name type in the query ie type since type is a reserved work in MySQL sytax.
How the hell do you escape ` marks anyway?
type is a reserved word in MS SQL. This means that it is used in other MS SQL operations. Similarly, you would run into trouble if you had a column named select or group
You can escape reserved words by using brackets: []

I have a edit form that has some checkboxes on it and I am having trouble getting the new checkbox values posted back to the db after it has been changed. So far the form will load in the current values in the db but after you click different check boxes it wont update the change in the db. Any help would be appreciated.

You are using SQL database from php and using mysqli_query() function to insert which would definitely not work. You have to use PDO. to access SQL database.

In this case, some of the below possibility will cause this issue.
Input type is mismatch with column data type in database table.
Required parameter to be used to insert into the table.
One suggestion to ensure that there is no issue in INSERT query. Just print the insert statement in browser and execute that manually in DB.

do you need help displaying the submited form data? or more spesific display for print?
to display it you would just need to make a html page that displays it.

To display just the result and not the form, when you post to the same page you need to encapsulate the for code with an if statement.

Use a new page for the "action" part in the form. On that new page, simply echo the $_POST of values you want to display. If you plan on making some sort of page so that people can check their entries, you could store these $_POST-data in sessions.

Right, to get your saved results from the database I'm sure you're aware of how to do this.
For your radio buttons you're going to want to select your 'size' value from the database and use an if statement to determine which radio button will be 'checked'
forename/surname is a matter of simply getting the forename/surname from database and setting the appropriate input tags to the acquired values
You've already posted a technique for handling the combo/drop-down boxes
you'd perform a similar process for the 'type' checkboxes, compare your database values to the values of your checkbox input tags and if the value matches, set the checkbox to checked.
Next time please be more specific in what you are asking for help with and separate your code into segments that correspond with the files it is contained in.
