Basically, I want to do display it as shown here:

In the PDF, the data will come from the database.
I would like to display like pdf image No.02 But i have done like pdf image No.01.
How to get the pdf image like pdf image No.02.
How can I solve it? Please help.
Not overly familiar with fPDF. It looks OK for quick, simple stuff.
Have you tried using R&OS ezPDF Creator instead - not sure what state of development/maintenance it's in these days but I think it may give you more detailed control for the kind of output you want.
I've used it for years for bespoke PDF output from PHP.

I have tried fpdf and a few other libraries for a big job i have done a couple of years back. You might want to look at dompdf, the library takes html and converts it into pdf flawlessly (after a couple of tries to get it right, but still) - only thing is, read the docs - images (like your barcode there) must be linked with server path, i.e. /path/to/image/image.jpg
Basically it looks ok. Its just a bunch of little things to adapt. e.g. setting the font to bold is done via $pdf->SetFont('Arial','B',14); before the output statement and setting it back to $pdf->SetFont('Arial','',14); afterwards. See fpdf documentation for details.

I am working with a tool which lets user upload a .csv file.
That csv file contains an address column. I have to use the address from each row in another HTML template. That HTML template is like this
. After creating that template I then need to convert it into a PDF, store the PDF on a file server and give the user a link to the PDF.
I've finished the first two steps - csv upload and created complete template with address, but I'm stuck on how I can convert a template into a PDF.
I have looked into a few php-pdf libraries like fpdf mpdf. I'm facing a problem in creating pdf with html template.

I have used tcpdf in many cases, https://tcpdf.org/
Works well with tables, I have made receipts and accounting related stuff with it. Handle UTF-8 without problems, why it's my way to go.
Only downside is that code is bit long and complicated and it doesn't keep tables as tables in pdf and turns them to divs, so paddings and other styles might be bit trickier to do.
One way is to use webkit based HTML to PDF converter.
Pros are that it is easy to customize and style and to see in the browser how it will look and then you can be sure that it will look as same in PDF as well. You could use CSS and JavaScript as well to style and modify.
Cons are that it is hard to install it on the production server sometimes. But there are web services and APIs that get you covered.
For example one service is https://pdfapi.io. It is free to use. Only when your amounts get bigger, then it will charge like a cup of coffee.

We have a high-resolution PDF (for printing) which has some form fields on it. We would like to have an HTML form which submits to the PDF, which is then placed into the respective fields.

However, with that solution you only get an FDF file... And the demo does not work for me, opening the FDF file simply downloads another FDF file.
Since this PDF will be available to the public we would like to keep it as simple as possible. If we must open our original PDF and import this FDF file, we need a different solution (which I'm not sure is what the FDF file is for, since it didn't work).

The PHP solutions I have found so far are for creating a new PDF, which is not what I need. Our PDF is created with Adobe Illustrator (or a similar adobe product) and is high-res with embedded fonts, svg and image content.
The form elements are in place, we just need to get the data to there.

Since posting this question I have been utilizing FPDF on multiple projects where I needed to accomplish this goal. Although it cannot seem to "merge" template PDFs with the provided data, it can create the PDF from scratch.
One example I have used, I had a high resolution PNG for printing (similar to initial question) which we had to write the customer's name and today's date clearly in the center. I simply made the background of the PDF using FPDF->Image() and write the text afterwards using FPDF->Text().
It was very simple after all, you will need to look up the paper sizes to determine the X,Y,W,H of the image and then base your text fields relative to those numbers.
There was even a Form Filling extension, but I couldn't get it to work.
It seems as though I should answer my own question, although Visions answer may be better (seems to be deleted?). I used Vasiliy Faronov's link which was a comment to my main question: https://stackoverflow.com/a/1890835/200445
Here I found how to install pdftk and run a command to merge (flatten) my FDF and PDF files. I still used the "hacky" way to generate an FDF using Koivi's FDF Generator but it works for the most part.
One caveat is that some characters, like single and double quotes are not inserted correctly. It may be an issue of escaping the fields, but I could not find an answer.
Regardless, my PDF form generator is working, but anyone with a similar issue should look for a better solution.

I want to generate PDF from a PHP file that includes HTML controls like textbox, and textarea. I attached CSS in the same. I tried FPDF, DOMPDF and TCPDF, but still I don't get exactly what I want. How do I pass HTML controls with PHP variables and CSS to these libraries?
mpdf is another option that you could try.

Found another solution for it, TCPDF is a FLOSS PHP class for generating PDF documents. Looks more dominating library.
"PRINCEXML" is a good library (not completely free now).

If your meaning is to create a PDF file from PHP, pdflib will help you (as some other suggested).
Else, if you want to convert an HTML page in PDF via PHP, you'll find
a little trouble outta here.. For three years I have been trying to do it as best as I
can.
So, the options I know are:
HTML2PS: same of DOMPDF, but this one convert first in .ps
(Ghostscript), then, in whatever format you need (PDF, JPEG, PNG). For
me it is a little better than dompdf, but I have the same speed problem.. Oh,
it has better compatibility with CSS.

If I understand your needs correctly I don't think any PHP-PDF class would do that.
Mostly you could insert only text and images to a PDF file, so if you would want something that looks like an HTML element you would need to insert it as an image.
Usually just putting HTML doesn't mean all your elements would stay intact in the PDF . (Different world, after all)
http://www.fpdf.org/ is the site having a great HTML-to-PDF class which work well. I am using it, but you have to first study its functionality and then start.
I need to generate a pdf file by adding pages from an already generated pdf using only php or javascript.
I tried fpdf but could not find any appropriate function. Is there a method I could use to do this?
There is an extension that should allow this to work called FPDI
That being said, I've not had a ton of luck through the years using FPDF. I've always fallen back to DomPDF or the likes. Like anything Adobe touches, PDF is not the most friendly tool in the world for those of us charged to make them work.

As you can see the image is placed behind the big image.

When I printed it using PrimoPDF driver it came out just fine. Last image was easily laid over. So there must be a bug with the script you're using.
What do I suggest?
If you'd like to convert your pages to PDFs "on the fly" I suggest you either
contact script creator and inform them of a bug in the script
use a different script (I'd check out this question that can help you)
If you'd like to just provide PDFs of your page I suggest you install a PDF printer driver (like PrimoPDF that I'm using) and print those pages yourself and use those.

On a sidenote
Something's telling me that what you'd actually like to do is to create a PDF flyer/promo material or something. If that's actually what you're after I suggest you do that using some software that's meant for such a job. Microsoft Office Word will do, but you'll better off using some other. If it's a one page leaflet you could use Adobe Illustrator or CorelDraw. But if it's going to be an actual multipage document use something like Word or Adobe InDesign.
Word is probably something you can easily master. So go with that one.
