I'm using Select2 in a combination of dropdown menus. I have one menu for "Countries" and one for "States/Provinces". Depending on the country that is chosen, the "States/Provinces" dropdown changes in content. The states/provinces are pulled with ajax from a database and then displayed this way:

So far, so good. All the provinces change correctly, however when it initially loads, the Select2 shows "undefined" for the states dropdown, even though I have it set as
data-placeholder="Choose a Country..."
I'm assuming it could be because on loading, the country selected is "United States" and it populates a list of states but none of them is default or selected. Is there any other way to define a default value so that it doesn't show "Undefined"?
And another (but less important) problem is that when someone chooses "United States" for example, and then chooses "Arizona", if the person then changes to "Canada" as the country, the state of "Arizona" still stays but when opening the dropdown the provinces of Canada are selectable. Is there any way to return it to the default value temporarily when someone selects another country, until a province is chosen again?

It's common for other components to be listening to the change event, or for custom event handlers to be attached that may have side effects. Select2 does not have a custom event (like select2:update) that can be triggered other than change. You can rely on jQuery's event namespacing to limit the scope to Select2 though by triggering the *change.select2 event.

select2 has the placeholder parameter. Use that one

Don't use destroy.
Finally solved issue of reinitialization of select2 after ajax call.
You can call this in success function of ajax.
Note : Don't forget to replace ".selector" to your class of <select class="selector"> element.

It's, maybe, not so good, nice and precise solution, and maybe I still did not clearly understood, how it works and why, but this was the only, what keeps my select2 dropdowns, gotten by ajax, alive.

The solution:
Once the content is loaded via ajax you can no longer attack generically like eg ‘.select2’. Because now other elements have this class as the span generated by select2.
So after loading ajax you need to call a method to check if select2 is already instantiated and instantiate it individually.

The $linebreak is to get rid of the duplicate builder names from the list, which works a treat.
What I would like to achieve is a second selector that selects the regions available, dependant upon the builder that has been selected in the first option. I hope this makes sense????
The second query needs to look at the builder selected in the first selector and filter out just the regions that are in rows with the builder name form selector one.
Please do say if you need further information, I'm not great at explaining myself.
As you said you don't have experience with jQuery or Ajax, I'll post my answer with as many comments as possible. I will assume that you have two select dropdowns in your page.
The first one contains the builders, so it will have id="builders".
The second one contains the regions, so it will have id="regions".
From what I understand, the first select will be exactly the one you posted in your question, generated server-side (by PHP). I only ask that you please make a slight change on it, making each option value be equal to the builder's database ID, and not its name (unless the builder's primary key is their name, and not an ID). This will make no difference for the final user but will be important for our jQuery solution. The second one will be empty, as the idea is to fill it dynamically with the regions related to the builder selected in the first dropdown.
Now let's get to the jQuery code:

I am using the extension Simple Configurable Products (SCP) for Magento version 1.8.1.0
What I am trying to achieve is have a configurable product (using SCP!) that displays the different options as radio buttons instead of a dropdown (default). Eventhough this does sound quite easy, I couldn't find a proper solution so far (there are a LOT of suggestions out there, none of which seems to work).
I would love to solve this on the backend, even though I will at this point appreciate working front-end solutions just as much.
What does work in the configurable.phtml is spitting out the different custom options, set for the product, which looks something like this:

Inchoo's plugin conflicts with SCP
Attribute Icons is in beta state and does't work either
Magento-Configurable-Products-Radio-Select does also not keep SCP-Funcionality
I know this is a very specific question and that it is a possible duplicate of this, however there might even be working solutions for v1.7 that are not working anymore in v1.8 and I have a feeling that this might be highly interesting for many others.
Edit: Just to elaborate on what SCP does a little more:
It allows you to have a configurable product that can have dependencies between options. This works by having a single product for every possible option. This way you can for example have the price go up, depending on material and size of a product. So one size can have different prices depending on the material and then a new price range if the size goes up. The GitHub site also gives a better understanding, just in case this was way confusing.
If you want to install SCP, make sure to install the fixes that are provided in the pull requests of the repo.

So this is what I ended up using. It does not change any code in the configurable.phtml or SCP core code. Instead I just make a call to skin/frontend/base/default/js/scp_product_extension.js to update the options and prices. This has to happen clientside, but so does most of the SCP logic apparently.

First function just turns the default <select> elements into radio buttons. <select> elements are hidden with CSS as we don't want the options to appear twice. Next is just listening for the click events on the radio-buttons and basically synch the clicks to the <select> elements. When the select element has changed, you just have to call the spConfig.configureElement(el); function where el is the corresponding <select> element.
If there is a second <select> element with dependencies to the first, this one will have to update as well when a radio-button is clicked.
Hope that gives others a clue, who attempt to do the same.
I have two select box and the second select box value is dependent on the first select box. So basically what I am trying to do is that after a user makes a selection on the first select box, it will then store this value of the first select box as a variable. Then it will query my database to populate the second select box, based on the first selected value. Question is, how do I pass in the var I have in the first select box to PHP? I've read other post and people said it's impossible, but if it is then how do people do this? AJAX?
If I understand correctly, then yes, using AJAX is really your only choice.
Indeed, you can, with AJAX, call something like 'getSelectionData.php?data=' + select1.value, which returns a JSON array of data based on the selection in the first select box. You then parse the data and enter it into the second select box.
Add a onchange event listener to the first select box:

This particular code is not tested, I've just written it. But it usually works fine to me this way.
You will have an onchange event on the first <select> that will query the server using Ajax with the value of the selected <option> that will return the <option> elements with which to populate the 2nd <select> element.

For my code, my drop down lists are initiated on the original page, via

This function is taking the value, equating it to country, then querying MySQL, and setting the results where id=province, and creating cascading dropdown lists. This is obviously via Ajax.
So, when $_REQUEST['province'] is set, then the Province dropdown list gets populated with all provinces from the country to which it belongs, etc.; i.e.;

Your code doesn't seem to make a lot of sense. The particular thing that worries me is that you say ajax is loading one item at a time?
Perhaps something like this. A country select tag like...

the PHP page that receives this AJAXed request would reply with a JSON object containing all of the province values, or a delimited list.
once the Javascript showRecords function receives the responce from the PHP page, it would add each of these options to the correct select tag. Once finished, it would set the default value to whichever option it wants by something like the following:

first is, if bydefault you have the select province like this

I have this webpage. It has a page called "services.php". I have several buttons (made of classes), that belong to different "package" prices i offer.
I want the links that say "Select" to autofill a form in another page, or alternativly in a popup form in the page..

When link is pressed autofill form (in this or other page) with the type of package they chose. Only text autofill
What you seem to be asking is 'loading' a page pre-filled with specific information, you can do this a number of ways, either by utilizing javascript (like jQuery for instance). Or using your PHP, make links that pass variables (say a flag or a reference to pre-fill the fields -- if you want a popup or next page, etc).
Your url would like like the following for the button that a user presses (button would be a simple http link):
http://mywebsite.com/prefill.php?user=bob&package=2
This would have the values bob as the user that requests it (you can reference an id for user info here as well), and package=2 to designate your package options.
Then on the prefill.php page, you would have something that checks for:
$user = $_GET['user'];
$package = $_GET['package'];

if i understood your problem, you want to fill some input fields with information when the user clicks on some links
i can think of 2 ways of doing this : either have the links point to a page like services.php?req=package1 (or any other url you want) and on that page generate the input fields with the information you need (set the default values in the fields with the ones you want), or, use javascript to change the values of the forms without changing the actual page (either via ajax or predefined values)
for javascript you can use the jQuery framework, it has a pretty extensive community of enthusiasts and plenty of examples to get you started with it.
an example for your case would be

replace btn1 with the id of the first button or link you have, input1 with the id of the first input in your form, and value with the value you want
I just did this myself. My solution was with jQuery. Just assign an id to your link. The first ID in the code is the link id and the second is the id for the input element you want to populate.

I've ran several time into the same issue, so I had to write my own script doing this. It's called Autofiller and its pretty simple but does great job.
Here is an example
http://example.com/?autofiller=1&af=1&pof=package&package=package1
So basically it takes several parameters to init the script:
autofiller=1 - init AutoFiller
af=1 - Autofill after page is loaded
pof=package - Find the parent form element of the select with name attribute package. Works also with input form elements.
package=package1 - Will set the select element's value to package1
