I want that when the submit button is clicked the success line comes as a dialog box on the page instead of a new page.
Unfortunately if this cannot be done then is there a way to stylize the text (success line) that comes after the submit button is clicked?

Yes it is possible.
1) create a hidden div on your form
2) On the main form create a link to your ajax call. Include the record key info on the link you wish to transmit to the web server/data server that you wish to have returned.
3) In your attached .js file create the ajax call and response. In the ajax call query send the key for the lookup to your server side response php app.
4) On your server side create a php response app
5) Your server side app looks up the record information and transmits back via a simple "echo" command.
6) Your client side app will then receive the information. In the response section of your ajax call add the information retrieved to the hidden and make the visible.
7) add a "close" link on the popup to rehide the popUp.
I would recommend that you send the information via JSON. This is very easy on each side client and server you just add a "encode" and "decode" line for each.
Since this is a lot of sample code to display, if there are parts of this you need help with, just post those specific questions for the parts of code you need help with.

Require ticked checkbox for php form submission

I do know php form validation is touched on often here, but I'm a complete php newbie and I'm just sort of hacking away at a php form script I found here, so I'm not sure how to proceed here with the two things I require.
I would like my form to require that a Terms & Conditions box is checked before it will submit (and if the box is not checked, to return an error message in the way this script processes error messages).
I would also like the fact that the Terms & Conditions box was checked to be emailed to me with the rest of the form data.
The salient bit of the html that I've tried is:

With what I've added, the form now throws a 500 Internal Server Error. I was able to successfully add and remove input fields to and from this php form, but I have no idea how to require it to have a checkbox filled in before it will submit (plus have it return the error message if it's not filled in) and have it email me, with the rest of the data, that it has been filled in.
If you want to verify the checkbox is 'ticked' (checked) before the form is send to the server, you will need some javascript.
A very (very!) crude check would look like
<form onsubmit="return this.elements['terms-and-conditions'].checked;">
This will (silently, so without feedback) prevent form submission if the 'terms-and-conditions' checkbox is not in a checked state.
As said, this is a very crude prevention, not at all usable. Please see this topic for more elaborate checks and solutions
As to why the server raises an 500 Internal Server Error:
You cannot have a variable with - characters in them, so $terms-and-conditions raises the parse error.
Consider using underscores instead, for example $terms_and_conditions or $termsAndConditions

I have created my first website with Bootstrap but having a problem with my contact form because it doesn't actually send the emails to my email address.
I'm not sure what is going on, because I have included my email address and have specified that it needs to be sent 'to' it.

How I can correct this so that it will function correctly?
HTML doesn't send emails. It can only submit form data to a script on a server written in another language like PHP. PHP only runs on a server, it won't run in the browser.
That PHP script (at www.yoursite.com/submit.php for example) in turn then takes that information and sends the actual email.
The location of the script you want to submit the form to is added under the action attribute of the <form> element. See the MDN or w3Schools pages about forms and actions for more details

A alert box should appear if there occurs any error below while submitting the form. And there should also appear a message box if the form is submitted correctly.

If you choose the latter method, then don't use the <input type="submit" /> but <input type="button" onclick="yourfunction();return false" />. The last bit (return false) is actually quite important.
