Im new to php, i have a working contact form, but I would like to add the senders name into the body of the email I receive, this is what I have tried -

It is better to use either $_POST and $_GET than $_REQUEST, and you are using $_POST for the body line, so I assume your data are sent using post.
When you receive any user input, you definitely have to test it. You can use filter_input to do that.

I would also advise not to use the given email address as sender, because the message could then be considered as a spam, coming from a non related smtp server...

I have it live and attempted to test send myself an email and I get a basic error (HTML Error 500) and no email, it changes the URL to "contactform.php" as expected. I followed the tutorials to the T, what am I doing wrong!

I've found some similar questions to my query, but none that fully match my situation.
I had a basic mail() PHP setup for use on the contact form on my website but, turns out this is blocked on my hosting server to avoid spam/abuse.
The web host recommends using SMTP as a workaround, but the tutorial they link to doesn't seem to provide a working solution for me!

If your form is in an email, I think you can't post data (maybe I'm wrong). But you can send it to your script with method get.

$name is my problem. I've I have it in, the email comes from hostmaster#domane.com, If I delete it, everything works fine. But I wan't the name to be sent to me. How?
Or should I do it completely different?
Also, if you leave all the fields blank, the "user" doesn't get any error message, and a blank email is sent to me.

Michael Berkowski is correct. What you'll need to do is add the name to your message's body (not in the sense of the input name= attribute, rather the body of the email).
Something like this:

You should read the mail function documentation on php.net.
Have a look at the function signature:
bool mail ( string $to , string $subject , string $message [, string $additional_headers [, string $additional_parameters ]] )
Now you're placing $name as the "$additional_headers" argument. You should pass $name and any extra relevant data in a $message argument instead.
Having that said, here's the correct code to send a message:
$sent = mail($to, $subject, "A message from $name: $message", $headers);
You should read more about how email messages are constructed. Instead of just putting a user defined message in there you probably want to specify some email headers, containing a more beautiful FROM: and the like...
